1.   Student Name
2.   Grade
3.  EMAIL ADDRESS
4.   Parent name
5.   "Tentative" project name
6.   Include in subject of email SCIENCE FAIR SIGN UP


If you do not receive a response, you are NOT
pre-registered. Email me again.

STEP TWO

Mail $4.00 registration fee per family NOT per person to:

This fee is NOT to make profit. It covers the ribbons,
certificates and ink.

Once fee is received you are completely registered.
THERE ARE NO REFUNDS!!!!

If we do not have a minimum of 35 registered
by January 15, the MTHSF will be cancelled,
and money will be returned.
